POSITIVE ASSOCIATION BETWEEN INTRINSIC PHOTOSENSITIVE RETINAL GANGLION CELLS AND RETINA NERVE FIBER LAYER IN GLAUCOMA.

OBJETIVO:
To assess the integrity of intrinsic photosensitive retinal ganglion cells (ipRGCs) using the pupillary light reflex in patients with glaucoma.

MÉTODO:
76 eyes of 38 patients with glaucoma and 36 eyes of 18 volunteers were included. Patients were tested in the dark and the light stimuli were generated by the Ganzfeld. In order to preferentially stimulate the intrinsic photosensitive retinal ganglion cells, we used a 1 second duration 470 nm flash, with 250 cd/m2 intensity, alternating with a 1 second 630 nm flash. Visual functional evaluation was performed with standard automated perimetry (SAP) and Frequency Doubling Technology (FDT) Matrix. Structural analysis was done by Optical coherence tomography (Cirrus OCT). We analyzed the correlation the sustained responses mediated by the intrinsic photosensitive retinal ganglion cells and the structural and functional visual changes in glaucoma patients.

RESULTADOS:
The mean (± standard deviation) age of the glaucoma and volunteers groups was 60.5 ± 11.2 and 56.2 ± 7.5, respectively (P = 0.052). There is an association between the average retina nerve fiber layer (RNFL) thickness in OCT and the pupillary response variable for the sustained response to blue flash and no association for SAP and FDT. (P = 0.024, P = 0.978 and P = 0.429, respectively) (GEE by SPSS 17). There is a strong association between the severity of glaucoma disease (Hodapp-Anderson-Parrish score) and the sustained response to blue flash (P = 0.006).

CONCLUSÕES:
There is a correlation between the mean RNFL thickness and the pupillary response. This RNFL reduction can be related to the decrease in the number of the ipRGCs in glaucoma patients.
